Living at Home Assessment, Information & Referral

JCS is able to undertake client assessments and regular reviews of the individuals holistic support needs, including facilitation of referrals to allied health organisations in order to access home safety, personal alarm assessments and health, falls & prevention information and advice.

These services are available as part of our subsidised Commonwealth Home Support Program services (CHSP) or privately funded services.

Person Centred Support Planning & Coordination

JCS is able to coordinate the development of an individualised support plan that promotes functional, social and emotional needs, as well as opportunities for meaningful social participation, social connectedness and life enjoyment.

Person Centered Support Planning is available as part of our subsidised Commonwealth Home Support Program services (CHSP) or privately funded services.

Advocacy

JCS can assist with information and support to promote and increase the your control over goods and services, overcome barriers that restrict opportunities, protect human rights and ensure a better quality of life, including referral to external advocacy organisations.

Advocacy services are available as part of our subsidised Commonwealth Home Support Program services (CHSP) or privately funded services.

Social Support and Shopping

Our team of reliable Home Support staff work alongside you to assist with tasks such as meal preparation, shopping, banking, paying of bills and accompanying to appointments & social visits in order for you to build resilience, confidence and maintain your independence as much as possible.

Social support & shopping services are available as part of our subsidised Commonwealth Home Support Program services (CHSP) or privately funded services.

Domestic Assistance

Our Home Support staff are available to work with you to assist with basic and essential household cleaning tasks in order to build your confidence and maintain capacity as much as possible and to remain living as independently as possible.

Domestic assistance services are available as part of our subsidised Commonwealth Home Support Program services (CHSP) or privately funded services.

Personal Care

Our professional care team can provide assistance or supervision with personal hygiene and daily self care tasks including working alongside the individual to demonstrate techniques to improve your capacity for self-management and to assist to build confidence in the use of equipment or aids.

Personal care services are available as part of our subsidised Commonwealth Home Support Program services (CHSP) or privately funded services.

Transport

Dedicated volunteers are available to provide reliable and safe transport for you to attend medical appointments, visit family in nursing homes and hospitals and to local social functions to assist you to remain independent and stay actively involved in your community.

Transport services can be provided as part of  our subsidised Commonwealth Home Support Program services (CHSP) or privately funded services.

Home & Garden Maintenance

Our team can provide assistance with small tasks such as changing light bulbs smoke detector batteries and mowing of lawns, edge trimming, trimming low bushes, branches and weeding to ensure that you are living safely in your own home.

Home & Garden Maintenance services can be provided as part of  our subsidised Commonwealth Home Support Program services (CHSP) or privately funded services.

Respite Care & Carer Support

Respite is designed to give a carer a break from what they are doing in their caring role. Our team of Home Support staff are available to provide Respite in the event of an unforeseen carer emergency.

Our Respite care services also enable carers to have a break to ensure that they do not burn out. Examples include going out socially, a special event or medical appointment, a family engagement, or just to have some time out from the caring role.

Respite Care and Carer Support services are available as part of our subsidised Commonwealth Home Support Program services (CHSP) or privately funded services.

The Aim of our In Home Support Services

To provide basic home and community support services so that frail older members of the community can continue to live independently in their own home and community. Our services provide assistance at home, support in getting out into the community, and a break for carers. 

Eligibility to Access our In Home Support Services

Our services are prioritised to meet the needs of older people (over 65) and their Carers who are unable to manage at home without assistance due to frailty or disability.

While our services are targeted to members of the Jewish community, we also welcome and encourage referrals from  other individuals over 65  who require assistance with home and community support to  ensure independence is maintained.

Making a Referral

PRIVATE FUNDED SERVICES

For individuals wishing to access our services on a private/full fee basis, please contact our Home & Community Support Team for further information and to organise an introductory home visit.

CHSP FUNDED & SUBSIDISED SERVICES

For individuals wishing to access subsidised Commonwealth Home Support Program (CHSP) services, you may prefer to complete the online referral form through My Aged Care or you may contact JCS directly to facilitate this for you.
